Who may not be able to join:

  • People who are waiting for a kidney from a living donor
  • People who have severe problems with memory or thinking

  • People who have been hospitalised more than 4 times in the last 12 months
  • People who are currently receiving nursing care, physiotherapy, or occupational therapy at home
