- People who had at least one blood pressure reading in the emergency department (ED) with the top number at 160 or higher, or the bottom number at 100 or higher

- People who own a mobile phone that can send and receive text messages
- People who are expected to be sent home from the emergency department (not admitted to hospital)
Who may not be able to join:
- People who are critically ill
- People who are unable to read English
- People who are currently incarcerated
- People who are pregnant
- People who have a pre-existing condition that makes it unlikely they would be able to complete a 6-month follow-up (confirm with trial site)
