Who might be able to join this trial:
Patients:

- People who have been newly diagnosed with multiple myeloma, whose multiple myeloma or aggressive B cell lymphoma has come back or stopped responding to treatment, or who need a change in their treatment plan

- People who are willing to attend at least one palliative care consultation visit with the patient
Healthcare providers:
- Specialist doctors, fellows, clinical nurse specialists, or clinic nurses from the outpatient blood cancer or palliative care team at the trial site
- Staff members who have been working in their clinical area for at least 12 months
Who may not be able to join:
Patients:
- People who do not have enough English reading or writing ability to complete the study tasks
- People whose doctor has determined they have poor cognitive (thinking and memory) function
- People who are already receiving care from a palliative care team at the trial site or elsewhere
- People who are not receiving ongoing follow-up care with the blood cancer specialist team at the trial site
Caregivers:
- People who do not have enough spoken or written English ability to complete the study tasks
- People whose patient is not participating in the study
Healthcare providers:
