Who might be able to join this trial:
- People who have been diagnosed with Parkinson's disease based on established medical guidelines
- People aged between 21 and 89 years old
- For the MRI-guided focused ultrasound (MRgFUS) treatment group: people where at least one of the three main Parkinson's symptoms (slowness of movement, tremor, or muscle stiffness) is rated at a moderate level or above

- For the deep brain stimulation (DBS) treatment group: people who meet Taiwan's national health insurance requirements, which include having had Parkinson's disease for more than 5 years, showing a meaningful improvement in movement with levodopa, and experiencing motor complications such as medication wearing off, unpredictable on-off periods, levodopa-related involuntary movements, or tremor that cannot be controlled by medication
Who may not be able to join:
- People with moderate-to-severe heart failure (classified as Functional Class III or higher) or advanced cancer that has spread to other parts of the body
- People whose Parkinson's disease has reached the most severe stage (stage 5 on the Hoehn and Yahr scale), meaning they are confined to a bed or wheelchair and require full assistance
- For the DBS group: people with significant dementia or major depression

Electroencephalogram recording before and after the tDCS session; Electromyography recording before and after the tDCS session; Functional magnetic resonance images examination before and after the tDCS session; Change in Unified Parkinson's Disease Rating Scale (UPDRS) Part III before and after the tDCS session UPDRS contains four parts, the third part of which is reported in this outcome.
