Who might be able to join this trial:
- People who identify as Black or African American
- People who have been diagnosed with high blood pressure (hypertension)
- People aged between 21 and 85 years old
- People whose blood pressure is not well controlled, meaning one of the following applies: their two most recent blood pressure readings taken at a regular clinic (not urgent care or emergency) were 140 or higher on the top number; OR their most recent reading was above 160 on the top number; OR their regular doctor referred them for uncontrolled high blood pressure with at least one reading of 140 or higher in the past 12 months
- People who receive their regular medical care at one of the clinics taking part in this trial
- People who are able to speak English
Who may not be able to join:
- People who are currently pregnant or planning to become pregnant during the study period
- People who live in a long-term care facility or hospice, or who have a terminal illness with a life expectancy of less than one year (note: people with stable long-term conditions such as managed liver disease, COPD, or heart failure are not excluded)
- People whose kidneys are functioning at a very low level (eGFR below 30) or who are currently on dialysis (note: people with a working kidney transplant are not excluded)
- People who are unable to give informed consent or take part in study activities, such as those who cannot participate in phone check-ins or have their blood pressure measured in at least one arm
- People who are currently taking part in another blood pressure management program
- People who plan to move away from the area within the next 12 months
- People who identify as Hispanic
