Who might be able to join this trial:

  • You are receiving pregnancy care at a Massachusetts General Hospital (MGH)-connected obstetrics practice (note: people attending the MGH HOPE Clinic are not eligible, as that clinic has a separate care model)
  • You are currently pregnant (with a living baby at 24 weeks or more of pregnancy) or have recently given birth to a living baby at 24 weeks or more of pregnancy
  • If you have already given birth, your baby is currently alive at the time of signing up
  • You have been diagnosed with at least one of the following conditions, recorded in your medical notes: high blood pressure (chronic), high blood pressure related to pregnancy (such as pre-eclampsia), Type 1 or Type 2 diabetes, gestational diabetes, obesity (with a pre-pregnancy BMI of 35 or above), or depression or anxiety
  • You have a primary care doctor or provider listed in your medical records
  • You have access to — or are willing to sign up for — the online patient portal used by your healthcare provider, and you agree to be contacted through it
  • You are able to read and speak English or Spanish
  • You are 18 years of age or older

  • Anyone who does not meet every one of the conditions listed above is not eligible for this trial
