- People who have high blood pressure (measured on three separate days, with a top number of 140 or higher, or a bottom number of 90 or higher), who are already taking blood pressure medication but whose blood pressure is still not well controlled according to the study doctor

- People who are able to care for themselves, communicate normally, and are willing to work with the study team throughout the trial and any follow-up visits
- People who live permanently in the city where the study centre is located
- People who are willing to take part and sign a consent form
Who may not be able to join:
- People with a serious advanced illness (such as those on dialysis or with liver failure), or whose life expectancy is estimated by the study doctor to be less than two years
- People with a psychiatric condition, memory or thinking problems, communication difficulties, or who are otherwise unable to work with the research team
- People who have had a stroke or cerebrovascular disease, or who have severe liver or kidney problems, severe heart failure, cancer, or a history of organ transplantation
- People who are pregnant, breastfeeding, or planning to become pregnant within the next two years
