Who might be able to join this trial:

• Men who identify as African American or Black
• Adults aged 18 years or older
• People who are able to walk and move around independently
• People who have been diagnosed with Type 2 Diabetes for at least one year
• People who are currently under a doctor's care for their diabetes and who report having an A1C blood sugar level of 7.0% or higher in the past year
• People who are willing to take part in weekly physical activity sessions, exercise sessions, CBT (talk-based) sessions, group discussions, and in-person health check-ups
• People who have a reliable internet connection or unlimited mobile data to complete study tasks on a tablet, phone, or computer
• People who live in Wayne or Washtenaw County
• People who have reliable transport to attend in-person events

Who may not be able to join:

• People with stage 2 high blood pressure (as defined by a specific medical guideline called JNC VIII) (confirm with trial site)
• People who have had a recent heart attack or other cardiac event
• People who have recently had laser eye surgery for a diabetes-related eye condition
• People who have had a stroke
• People who have had a lower leg or foot amputation
• People with nerve damage in the feet or legs (peripheral neuropathy)
• People with severe lung disease requiring oxygen at rest
• People with serious heart failure (classified as Class III or IV)
• People whose health is currently considered medically unstable
• People currently experiencing ongoing chest pain at rest or unstable angina
• People with an uncontrolled irregular heartbeat that is affecting blood circulation
• People with an active heart lining infection (endocarditis)
• People with a severe narrowing of the heart's aortic valve that is causing symptoms
• People with current inflammation of the heart muscle or the sac around the heart
• People with a sudden tear in the body's main artery (aortic dissection)
• People with a recent blood clot in the lungs (pulmonary embolism or pulmonary infarction)
• People with a blood clot in a deep vein
• People with a physical condition that would make exercise testing unsafe
• People currently experiencing chest discomfort during physical activity, unusual shortness of breath, dizziness, fainting, or blackouts
• People who are currently receiving talk therapy (psychotherapy) from a mental health provider specifically for the treatment of depression — though people who are only receiving medication management from a psychiatrist for depression may still be considered eligible
