Who might be able to join this trial:
For clinicians (doctors and advanced practice providers):
- Attending clinicians who work on the general medicine or hospital medicine service at one of these hospitals: UPMC Presbyterian, UPMC Montefiore, UPMC Shadyside, UPMC Mercy, or UPMC Magee-Women's Hospital
- Clinicians in this group may include general internal medicine doctors, hospital medicine doctors, family practice doctors, internal medicine specialists, or advanced practice clinicians
For patients:
- Adults aged 65 or older who are in hospital under the care of a clinician taking part in the trial, and who are being sent home after their stay
- Patients who have had at least one change made to a heart- or metabolism-related medication at the time of hospital discharge — this includes starting a new medication, stopping a medication, or changing a dose; the types of medications covered include blood pressure medications, cholesterol-lowering medications, blood-thinning medications, or diabetes medications
Who may not be able to join:
For clinicians:
- Clinicians who are planning to leave their current clinical position within the next 3 months
- Clinicians who have fewer than 4 weeks of inpatient attending service scheduled within 3 months of being recruited into the trial
For patients:
- People who are not able to give informed consent or answer survey questions due to cognitive impairment
- People who are currently enrolled in hospice care
