Who might be able to join this trial:
- You are 18 years old or older and have been diagnosed with a type of rectal cancer called adenocarcinoma, located in the middle or lower part of the rectum
- Your cancer is either at an advanced local stage, or its position in the rectum means that surgery to remove the rectum and anus (called rectal amputation) is needed
- Scans (MRI) show your tumor is located below a certain internal boundary in the rectum, which defines it as being in the middle or lower rectum
- The lower edge of your tumor is less than 2 cm from a specific internal landmark (the pectineal line or anorectal ring), as measured by a camera examination of the rectum, physical examination, or MRI scan
- If you have middle rectum cancer, you may be included if your tumor has grown into or very close to the surrounding tissue layer (mesorectal fascia), or if your cancer is classified as a more advanced stage (cT4)
Who may not be able to join:
- Your overall health and fitness level means you are not well enough to tolerate the full course of treatment planned before surgery (called total neoadjuvant treatment)
- Your rectal cancer is a type other than adenocarcinoma
- Your adenocarcinoma has a specific genetic feature called microsatellite instability, which is identified through a laboratory test on your tumor tissue
- You have a middle rectum tumor that does not meet the specific criteria for receiving radiotherapy before surgery, based on how far it has spread and whether the surrounding tissue layer appears clear of cancer on scans (confirm with trial site)
