Who may be able to join
- Adults aged 18 or older.
- People who are able to carry out most daily activities (based on a standard activity scale called ECOG, scored 0–2).
- People who are willing and able to sign a consent form to take part in the study.
- People who have been diagnosed with metastatic non-small cell lung cancer (a type of lung cancer that has spread) with a specific gene change — either an EGFR mutation or ALK rearrangement — confirmed by scans and a biopsy.
- People in Group B of the trial who are eligible for a second round of treatment or a targeted drug treatment called a TKI (confirm with trial site).
- People who are willing to provide a blood sample for a specific type of cancer DNA test (ctDNA).
- People with any number of sites where cancer has spread, but with no more than 5 areas where the cancer is actively growing that do not need urgent treatment.
- People who have previously had radiation therapy near the areas of active cancer growth, if that applies to their situation.
- People whose cancer areas can be treated with a precise, focused type of radiotherapy such as SBRT or ablative therapy.
- People whose areas of active cancer growth have each been individually assessed using specific criteria for a condition called oligoprogressive disease (confirm with trial site).
- People who have stable cancer spread to the brain, as long as it is not causing symptoms and does not require steroid medication.
- People who have previously had radiation to areas of active cancer growth, as long as those areas are still not causing symptoms and can be treated with radiation again.
- People who have previously had standard (non-stereotactic) radiation for symptom relief, where the treated area has since progressed but is still not causing symptoms and does not need immediate treatment — this area may count as one of the 5 active cancer sites.
- People who are willing and able to complete quality of life and health questionnaires in English, French, or Spanish.
- People who are able to attend treatment and follow-up appointments.
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
- People who have more than 5 sites outside the brain where the cancer is actively growing.
- People who are pregnant.
- People who have cancer that has spread to the lining around the brain and spinal cord (leptomeningeal disease).
- People with serious health conditions that prevent them from safely receiving radiotherapy, such as ataxia-telangiectasia or scleroderma.
- People who have previously had radiation near an active cancer site in a way that would make it unsafe to deliver the precise radiotherapy used in this trial.
- People with psychological, social, or geographic circumstances that may make it difficult to follow the study requirements.
- People who have any other condition that the trial investigator considers would make participation in the study inappropriate.
