Who might be able to join this trial:
- Adults aged 18 or older who have atrial fibrillation or atrial flutter (in any form — ongoing, long-lasting, or coming and going) that requires blood-thinning medication
- People currently taking warfarin (with a target INR of 2.0–3.0) or one of the following blood thinners at a standard dose: apixaban, edoxaban, dabigatran, or rivaroxaban
- People who have a planned, non-urgent surgery or medical procedure coming up

- People who are taking blood thinners for a reason other than atrial fibrillation or flutter — for example, for a mechanical heart valve or a blood clot condition
- People whose blood thinner is prescribed at a non-standard dose or target level (confirm with trial site)
- People taking one of the newer blood thinners (not warfarin) whose kidneys are not working well enough to safely use those medications
- People with significant memory, thinking, or psychiatric difficulties that may make regular follow-up contact unreliable
- People who are not able or not willing to agree to receiving some care virtually (by phone or video rather than in person)
- People who have already taken part in this same trial for a previous surgery or procedure
