Who might be able to join this trial:
- People who have been confirmed through tissue testing to have cholangiocarcinoma (bile duct cancer), or who are strongly suspected to have it based on scans, internal camera examinations, and a raised blood marker called CA 19-9 above 100 U/L
- People who have a specific type of bile duct cancer near the liver (called perihilar cholangiocarcinoma) that cannot be removed by surgery, either because of where the tumour is located or because of the condition of the underlying liver
- People with a liver condition called PSC (primary sclerosing cholangitis) who had a previous surgery more than 24 months ago with no cancer spread to lymph nodes, no cancer left behind, and no major blood vessel involvement, and whose cancer has returned in the same area
- People whose cancer has not spread to distant parts of the body or to certain lymph nodes in the abdomen (near the aorta, coeliac region, or colon)
- People who are not considered eligible for a liver transplant under a specific set of medical guidelines called the Mayo Protocol (confirm with trial site)
- People who have a good general level of physical function, rated 0 or 1 on a standard medical scale (meaning they are fully active or have only minor limitations in daily activities)
- People who have already been observed and treated with chemotherapy and radiation therapy (a specific dose range of 30–50 Gy) for at least 6 months, and whose cancer has responded to or remained stable during that treatment
- People with PSC and significant liver problems that limit their ability to tolerate chemotherapy and radiation may be considered individually (confirm with trial site)
- People whose cancer involves certain blood vessels near the liver (the hepatic artery beyond a specific branch, or the portal vein without a blood clot in the tumour) may still be considered eligible
Who may not be able to join:
- People whose scans show the tumour is growing into the edges of where surgery would need to cut
- People whose tumour has grown directly into the head of the pancreas
- People whose cancer has spread to certain abdominal lymph nodes (near the aorta, superior mesenteric artery, or coeliac region)
- People who have had a tear or rupture of the lining covering the abdominal organs
- People who have lost more than 10% of their body weight in the past six months
- People with a BMI (body mass index) greater than 30 kg/m²
- People with another active cancer diagnosis, with exceptions for certain skin cancers (basal cell carcinoma) that were fully treated, or other cancers that were treated more than five years ago with no return — the trial's principal investigator makes the final decision on previous cancer history
- People with a known history of HIV infection
- People with substance use issues, or medical, psychological, or social circumstances that could affect their ability to take part in the trial or for results to be properly assessed
- People with a known allergy or sensitivity to a medicine called rapamycin
- People who have previously had cancer that spread to distant parts of the body
- People who are pregnant or breastfeeding
- People whom the investigator considers should not participate, for any reason
