Who may be able to join
- People who are 18 years of age or older.
- People who have given written consent to participate in the study.
- People who have been confirmed through laboratory testing (tissue or cell samples) to have a specific type of lung cancer called oligometastatic extensive-stage small cell lung cancer, where cancer has spread to no more than 3 organs and no more than 5 separate spots in the body (confirm with trial site for full staging details).
- People whose chemotherapy plan includes both cisplatin and etoposide.
- People who are expected to survive for at least 12 weeks.
- People with a physical functioning score (called an ECOG score) of 0 or 1, meaning they are fully active or able to carry out light activity (confirm with trial site).
- People who weigh more than 40 kg.
- People who have at least one measurable tumour spot (that has not previously been treated with radiation) that is large enough to be accurately measured by CT or MRI scan, according to standard measurement guidelines.
- People who have not received any anti-cancer treatment before.
- People whose organs and bone marrow are functioning well enough to meet the study's requirements (confirm with trial site for specific blood and organ tests).
- People whose heart pumping function (left ventricular ejection fraction) is at or above the normal lower limit of 50%, as shown on a heart ultrasound.
- Female patients who are either post-menopausal (no periods for 12 months with no other medical cause) or who have a negative pregnancy test result before joining.
- People who have completed the required staging tests (such as brain MRI, chest and abdominal CT scan, lymph node ultrasound, and whole-body bone scan) within approximately 2 weeks before enrolling.
- People who have brain metastases that are not causing symptoms or are stable after treatment, and who do not need steroid (glucocorticoid) medication, or have stopped taking it at least 3 days before the first study treatment.
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
- People who are directly involved in designing or running this study.
- People who have previously received the study drug being tested in this trial.
- People who are currently enrolled in another clinical trial that involves an active treatment (participation in a non-treatment observational study or a follow-up phase of a previous trial may be acceptable).
- People who have a medical reason that makes etoposide-platinum chemotherapy unsafe for them.
- People who have previously received radiation therapy to the chest area before starting systemic treatment.
- People who are currently receiving other cancer treatments such as chemotherapy, biological therapies, or hormonal cancer treatments (hormone therapy for non-cancer reasons, such as menopause hormone replacement, may be acceptable).
- People who have had a major surgical procedure within 28 days before the first study dose (confirm with trial site for what counts as "major").
- People who have previously had an organ transplant from another person.
- People who have been treated for aplastic anaemia or other blood disorders, or who have had a stem cell transplant.
- People who have a nerve or brain condition related to their cancer (called paraneoplastic syndrome) that requires treatment with steroids or immune-suppressing medications, or who are showing signs that this condition is getting worse.
- People with a current or past autoimmune or inflammatory condition (such as inflammatory bowel disease, lupus, rheumatoid arthritis, sarcoidosis, certain kidney diseases, Graves' disease, or similar conditions), with some exceptions — for example, people with vitiligo, hair loss (alopecia), stable thyroid conditions managed with hormone replacement, certain mild skin conditions not needing systemic treatment, celiac disease managed by diet alone, or those whose autoimmune condition has not been active for at least 3 years (confirm with trial site).
- People with ongoing serious health problems that are not well controlled, including active infections, lung disease, heart failure, uncontrolled high blood pressure, unstable chest pain, heart rhythm problems, severe bowel disease with diarrhoea, recent stroke or brain bleed within 3 months, or mental health or social conditions that could affect their ability to safely participate in the study.
- People with a history of another cancer, unless it was treated with the intent to cure and has shown no signs of returning for at least 5 years, or it was a type of skin cancer or pre-cancerous condition that was fully treated with no current signs of disease.
- People who have had cancer spread to the membranes surrounding the brain and spinal cord (leptomeningeal carcinoma).
- People with a history of a primary immune deficiency condition.
- People with an active infection, including tuberculosis, active hepatitis B (with a positive surface antigen test), active hepatitis C (with a positive RNA test), or HIV. People who have previously had hepatitis B but are considered cleared of it, or who have hepatitis C antibodies but test negative for active virus, may still be eligible (confirm with trial site).
- People who are pregnant or currently breastfeeding.
- People who the trial investigator considers to be unsuitable for the study for any other reason.
