Who might be able to join this trial:
- You are willing and able to make your own decision to take part in the trial
- You are being treated as an outpatient (not currently staying in a hospital)
- You are 60 years old or older
- You have been formally diagnosed with major depression (MDD) and are currently experiencing a depressive episode
- You have tried an antidepressant at an appropriate dose and it did not work well enough, or you were unable to tolerate two different antidepressants
- Your depression symptoms score at least 10 on a standard depression questionnaire (PHQ-9), indicating at least moderate symptoms
- You have not started or increased any antidepressant or antipsychotic medication in the 4 weeks before the study screening
- You are able to attend and follow the treatment schedule
- You pass a standard safety screening questionnaire for the brain stimulation treatment used in this trial (TMS)
Who may not be able to join:
- You have had a problem with substance abuse or dependence in the last 3 months
- You have a serious unstable physical health condition, as decided by a study doctor
- You are currently having thoughts of suicide with intent to act on them
- You show signs of dementia or significant memory problems, based on a short memory test
- You have ever been diagnosed with bipolar disorder (type I or II) or a psychotic disorder
- You are currently experiencing symptoms of psychosis (such as hallucinations or delusions)
- You have a diagnosis of OCD, PTSD (current or within the last year), an anxiety disorder, or persistent low-grade depression (dysthymia), and a study doctor decides that condition is more severe than your depression
- You have a personality disorder that a study doctor considers to be causing more problems than your depression
- You had electroconvulsive therapy (ECT) during your current depressive episode and it did not help
- You have already received repetitive TMS (rTMS) treatment during your current depressive episode (having had it in a past episode is acceptable)
- You have a history of epilepsy or seizures related to a brain lesion
- You have a metal implant in or near your head (such as aneurysm clips, brain stimulators, cochlear implants, or similar devices), not including dental work
- You have an active implanted electronic device, such as a heart defibrillator
- You have a significant hearing or sensory impairment that cannot be corrected and would prevent you from taking part in interviews
- You have significant abnormal results on laboratory tests, in the opinion of a study doctor
- You currently take more than a certain daily dose of a sedative medication (lorazepam 2 mg or equivalent), or any amount of anti-seizure medication
- If you are in psychotherapy, your therapy must have been stable for at least 3 months before joining, with no expected changes in how often you attend or what you focus on during the study
