- Adults over 18 years old who receive their regular (primary) healthcare at one of the 9 participating clinics and have had a visit with their regular doctor in the past year
- People who have been identified as having high blood pressure, based on at least one of the following: high blood pressure appearing on their active medical problem list, being prescribed high blood pressure medication in the past year, or having three separate high blood pressure readings recorded
- People whose high blood pressure is not currently well controlled, meaning their top blood pressure number (systolic) is above 140
