- People who have a heart muscle disease that causes thickening of the heart walls, unless it is caused by high blood pressure
- People with serious heart valve disease, including those who have a mechanical valve or have had any surgery on the mitral valve
- People with heart defects present from birth, including certain repaired holes in the heart
- People who have a blood clot present inside the heart
- People who had a heart attack or a procedure to open blocked heart arteries within the last 6 months
- People who had heart surgery within the last 6 months
- People who had a stroke or blood clot travelling through the body within the last 6 months
- People who had a blood clot in the lungs within the last 6 months, or who have high blood pressure in the lungs requiring ongoing treatment
- People who are not able to read independently
- People who are minors or who are under a legal guardianship or protection order
