Who might be able to join this trial:
- You have been diagnosed with at least one of the following: high blood pressure, type 2 diabetes, high cholesterol, chronic kidney disease, and/or metabolic syndrome.
- You are 18 years of age or older at the time of signing the consent form.
- You have reliable transportation to attend in-person exercise sessions.
- You have access to a device that can connect to the internet (such as a smartphone, tablet, or computer).
- You are comfortable with ordering groceries online, receiving food deliveries at home, and receiving nutrition advice.
- Your home address is eligible for Instacart grocery delivery (for example, you live in a regular home, not an institutional facility).
- You are willing to use a personal credit card as a backup payment method on Instacart if needed.
- You are able to speak English, in order to take part in the program and give your consent.
- You have been prescribed a GLP-1 medication by your doctor (a type of medication used for conditions like type 2 diabetes or weight management).
Who may not be able to join:
- You have a condition that affects how your body digests food or absorbs nutrients — for example, you have had surgery to remove part of your digestive system, have had certain brain surgeries affecting thinking, or have had weight-loss (bariatric) surgery.
- You have a mental health condition (such as schizophrenia, bipolar disorder, or severe depression) that is not well-managed or controlled with treatment, as decided by the study team.
- You have a family history of certain cancers, such as multiple endocrine neoplasia or medullary thyroid carcinoma (confirm with trial site).
- You have an active digestive illness such as Celiac disease, irritable bowel syndrome, or a condition causing long-term poor nutrient absorption.
- You have other medical or psychiatric conditions that the study doctor believes could put you at risk by participating.
- Your kidneys are not functioning well enough, based on a specific blood test result (eGFR below 60 — confirm with trial site).
- There is any other reason the study doctor believes you may not be able to fully participate.
- You have taken part in another nutrition or behavior-based program or trial in the past 3 months (such as a diabetes education program or bariatric program).
