Who might be able to join this trial:
- People who speak English or Spanish
- People who have been diagnosed with high blood pressure
- People who have been prescribed at least one medication to lower blood pressure (such as a water pill, calcium channel blocker, beta-blocker, ACE inhibitor, or ARB)
- People whose last two blood pressure readings at their regular doctor were 130/80 mmHg or higher
- People who have visited their regular doctor in the past 6 months
- People who have been identified as experiencing food insecurity based on a standard two-question screening tool called the Hunger Vital Sign
Who may not be able to join:
- People whose blood pressure is very high (top number above 210 mmHg or bottom number above 120 mmHg)
- People with significant memory or thinking difficulties, a major mental health condition that affects their ability to give consent, or a serious medical condition that limits life expectancy or requires active ongoing treatment (such as certain cancers)
- People who are pregnant, breastfeeding, or planning to become pregnant in the next 6 months
- People who are planning to move out of the local area within the next 6 months
- People who do not have safe, stable housing
- People who do not have the ability to store meals at home
- People who do not have access to a telephone
