Who might be able to join this trial:

• Adults aged 18 or older who have a reasonable level of physical functioning (able to carry out basic daily activities) and are expected to live for at least 3 months.
• People who have been diagnosed with advanced, metastatic pancreatic cancer confirmed through laboratory testing of tissue or cell samples.
• People who have at least one tumour that can be measured using standard imaging criteria.
• People who have not received any previous cancer treatment of any kind.

Who may not be able to join:

• People who have previously received any systemic cancer treatment, such as chemotherapy or radiotherapy.
• People who have taken part in another drug clinical trial within the past 4 weeks.
• People whose cancer may be treatable with surgery, unless a surgeon has assessed the situation and surgery has been declined by the patient and the trial investigator agrees to enrolment.
• People who have a moderate build-up of fluid in the abdomen that requires draining (small amounts of fluid seen on scans without any symptoms may be acceptable — confirm with trial site).
• People with active symptoms from cancer that has spread to the brain or the lining around the brain.
• People with a history of another cancer, unless it has been in full remission for at least 2 years, or it was a non-melanoma skin cancer, or it was a very early-stage (in situ) cancer — all of which must have been adequately treated with no signs of return.
• People with autoimmune conditions or immune deficiencies who are currently being treated with medicines that suppress the immune system.
• People who have a tendency to bleed or bleeding-related conditions.
• Women who are pregnant or breastfeeding; women of childbearing potential must have a negative pregnancy test within 7 days before joining the trial.
• People with substance abuse issues, or psychological or social circumstances that may affect their ability to give informed consent or follow the study.
• People who may be allergic to any of the study medications: epaltrastide (QL1706), lenvatinib, albumin-bound paclitaxel, or gemcitabine.
