Who might be able to join this trial:

  • People aged 14 to 19 years old
  • People who have been diagnosed with major depressive disorder, or bipolar I or II disorder, currently in a depressive episode without psychotic or mixed features
  • People whose current depressive episode has lasted at least 4 weeks
  • People who have tried at least one talking therapy or antidepressant medication that did not work well enough
  • People whose depression scores as moderate to severe on a standard rating scale used by clinicians (the GRID-HAMD-17)
  • People who are currently admitted to a hospital, or for whom a planned hospital admission would be appropriate

Who may not be able to join:

  • People who have ever been diagnosed with a psychotic disorder, anorexia nervosa, or bulimia nervosa, or who have been diagnosed with a substance use disorder (other than caffeine or tobacco) in the past year
  • People whose main source of distress is obsessive compulsive disorder or a personality disorder
  • People currently experiencing psychosis or a manic episode
  • People with a history of epilepsy or any other neurological condition that has involved seizures
  • People taking medications that are known to lower the threshold for seizures (confirm with trial site)
  • People who are pregnant or breastfeeding
  • People who have experienced severe headaches within the past year
  • People whose depression symptoms have improved by 25% or more between the initial screening appointment and the start of the trial
  • People who would not be able to follow the trial's required procedures

Feasible: 90% of the time, rTMS treatment will be started within 3 business days of a patient consenting during an existing inpatient stay or within 3 business days of the start of an elective inpatient stay; Feasible: 90% completion of weekly Hamilton Depression Rating Scale during treatment by clinician; Acceptable: 25% or lower drop-out rate from completing treatment; Accessible: 90% of the time, suitability assessments for this treatment pathway will be completed within 2 business days for exiting inpatients and within 2 weeks for outpatients; Demand: at least 30 youth will consent to bein...
