Who may be able to join
Before surgery (initial registration):
- People aged 20 to 79 years, of any gender.
- People whose CT or PET-CT scan shows a single lung nodule measuring 2 to 3 cm across, suspected to be non-small cell lung cancer, with no signs of spread to nearby lymph nodes, and not located in the middle lobe of the lung.
- People whose scan shows the nodule is mostly solid (more than half of it appears solid on the CT scan).
- People with good enough lung function to safely undergo either a segmentectomy (removal of part of a lung) or a lobectomy (removal of a whole lung lobe).
- People with a general health status rated 0 to 2 on a standard medical scale (meaning they are able to carry out daily activities, with at most some limitations).
- People who are willing to take part, have signed a consent form, and are able to attend study visits and follow study requirements.
- People who have not had surgery on the same side of the chest before, and whose examination by keyhole camera confirms they meet the surgical requirements.
- People with no history of chemotherapy or radiotherapy — including for other cancers — or people for whom more than 5 years have passed since completing a short course of chemotherapy around a previous surgery. People with a history of hormone therapy, or currently on hormone therapy, may also be eligible.
- People whose recent blood and organ function test results (taken within 28 days before registration) all fall within the required ranges: sufficient white blood cells, haemoglobin, and platelets; liver enzyme and bilirubin levels within acceptable limits; and kidney function within the required range (confirm with trial site for exact values).
During surgery (secondary registration):
- People whose tissue sample tested during surgery shows a type of lung cancer called invasive lung adenocarcinoma, graded 1 or 2 on the IASLC scale (meaning less than 20% of the cancer cells are of a more aggressive type).
- People whose surgical margins (the edges of the removed tissue) are clear of cancer cells during the operation.
- People for whom the surgeon confirms during the operation that both types of surgery (segmentectomy and lobectomy) are safely possible, with no severe scarring or inflammatory changes around blood vessels or airways.
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
Before surgery:
- People with an active bacterial or fungal infection confirmed by imaging or laboratory tests, with a fever above 38°C.
- People with more than one active cancer at the same time or at different times (not including certain very early cancers considered fully treated by local treatment).
- People with seriously reduced heart, liver, or kidney function based on specific test thresholds (confirm with trial site for exact values).
- People who have another type of cancer or a blood disorder at the same time.
- People who are pregnant, planning to become pregnant, or breastfeeding.
- People who have received any cancer treatment before surgery, including chemotherapy, radiotherapy, ablation, or targeted therapy.
- People who have taken part in another cancer-related clinical trial within the past three months.
- People whose CT scan shows the nodule is mostly non-solid (less than half appears solid on the scan).
- People whose scans or lymph node tests before surgery suggest cancer has spread to nearby lymph nodes.
- People whose tumour is positioned too close to the central part of the lung to allow a segmentectomy.
- People considered unsuitable for the trial by the investigating doctor.
During surgery:
- People whose tissue sample tested during surgery shows a more aggressive cancer grade (IASLC grade 3, meaning 20% or more of the cells are of a higher-grade type).
- People whose tissue sample during surgery shows the cancer is at a very early, non-invasive stage (in situ carcinoma or minimally invasive adenocarcinoma).
- People in whom distant spread of cancer, or fluid around the lungs or abdomen, is found during surgery.
