Who might be able to join this trial:
- People aged 40 or older who have had Type 1 diabetes for more than 10 years, diagnosed before age 30 (or after 30 with specific supporting evidence such as a diabetic emergency at onset or positive antibody tests).
- People who have at least one of the following: kidney disease (based on specific urine or blood test results), a history of heart attack, stroke, or chest pain from the heart, a history of heart failure, a body mass index (BMI) over 35, or a calculated 5-year heart disease risk above 10% using a specific diabetes risk tool.
- Women who are able to become pregnant must be using a highly effective form of contraception throughout the study and for at least 2 months after finishing the study medication — accepted methods include IUDs, hormonal contraception (such as pills, implants, patches, vaginal rings, or injections), or they must have reached menopause, had both ovaries removed, been surgically sterilised, or had a hysterectomy at least 12 months before screening.
- People who are able to understand information about the study and communicate with the research team.

Who may not be able to join:
- People who have Type 2 diabetes, a rare inherited form of diabetes (MODY), or diabetes caused by another medical condition.
- People with a history of pancreatitis (inflammation of the pancreas).
- People with a BMI below 18.5 (considered underweight).
- Women who are pregnant, breastfeeding, planning to become pregnant, or not using adequate contraception.
- People with a known or suspected problem with alcohol or recreational drug use.
- People already taking part in another clinical drug trial.
- People with advanced kidney disease classified as Stage 5.
- People with an active cancer diagnosis (for a specific part of the trial called the MICRON study).
- People already taking certain medications — specifically SGLT inhibitors, GLP-1 receptor agonists, or finerenone — at the time of joining (for the MICRON study) (confirm with trial site).
