Who might be able to join this trial:
- People who are between 16 and 19 years of age
- People who have had type 1 diabetes for at least 12 months
- People who have recently had an HbA1c (a blood sugar level measurement) result of 7.0% or higher
- People who currently receive diabetes care at a Seattle Children's Hospital Diabetes Clinic in Bellevue, Everett, Federal Way, or Seattle
- People who are able to complete written surveys
- People who will be able to continue receiving medical care in Washington State for the next 2 years
Who may not be able to join:
- People who have already had a clinic visit through the pilot program called the AHEAD clinic
- People whose most recent standard diabetes care visit was with a provider who is currently part of the AHEAD program
