- People who have been diagnosed with chronic kidney disease (CKD) at stages G1, G2, G3a, G3b, or G4, and who are not currently being considered for dialysis treatment
- People who have uncontrolled type 2 diabetes, cardiovascular disease, and/or obesity
- People who are willing to take part in the study and sign a consent form agreeing to participate

- People who have chronic kidney disease at stage G5, or stage G4 where dialysis is being considered as a treatment option
- People who have a neurological or psychiatric condition that may prevent them from fully understanding the potential benefits and risks of the study (confirm with trial site)
- People who decline to take part, or who have previously agreed but then withdrawn their consent
