Who might be able to join this trial:

• You want to keep your anus (avoid a permanent colostomy bag) and are willing to complete the full course of pre-surgery treatment offered in this trial.
• You are between 18 and 75 years old (any gender).
• Scans and camera tests have shown your rectal tumor is within 10 cm of the anal opening, has not spread to distant parts of the body, and any affected lymph nodes are only in the immediate area around the rectum.
• A tissue sample (biopsy) has confirmed your cancer is rectal adenocarcinoma, and genetic or protein tests show a specific result called MSS, MSI-L, or pMMR (confirm with trial site if unsure what your results mean).
• You are generally fit and able to carry out everyday activities, with little or no limitation from your illness (this is measured on a standard scale called ECOG 0–1).
• You have not had any cancer treatments, immunotherapy, or radiation to the pelvis before joining.
• Your blood, liver, and kidney test results fall within the ranges required by the trial (confirm specific numbers with trial site).
• You are willing to sign a consent form, attend follow-up appointments, and follow the trial's requirements.

Who may not be able to join:

• You have been diagnosed with another cancer (either currently or in the past), in addition to rectal cancer.
• Scans show your cancer has spread to other parts of the body beyond the rectum.
• Scans show suspicious lymph nodes in areas further away from the rectum, such as in the pelvis (e.g., iliac lymph nodes).
• You urgently need surgery right away because of a blocked, perforated, or severely bleeding bowel.
• You have serious other health conditions and your life expectancy is estimated at 5 years or less.
• You have a known allergy to any of the medications or substances used in this trial.
• You have taken medicines to suppress your immune system (including steroid medicines used for this purpose) in the 30 days before starting the trial.
• You have taken part in another clinical trial or received an experimental treatment within the 30 days before being assessed for this trial.
• You have issues such as heavy alcohol use, drug misuse, serious mental health conditions, or other major illnesses that could affect your ability to take part safely.
• You have a condition that weakens your immune system, such as HIV infection.
• You fall into certain groups that require additional protections, including people with psychiatric disorders, cognitive difficulties, serious illness, pregnancy, being under 18, or being unable to read.
• The trial doctor considers you unsuitable for any other reason (confirm with trial site).
