- You have an upcoming appointment with a primary care doctor or heart specialist at NewYork-Presbyterian/Columbia that is a regular visit (not just a lab test or procedure)

Who may not be able to join:
- You do not speak English or Spanish
- You have a history of a serious mental health condition, such as bipolar disorder, schizophrenia, long-term or recurring depression, or a past suicide attempt or self-harm
- You have been diagnosed with dementia
- You have had a combined mental health and medical care visit in the last 3 months, as shown in your medical records
- You are already taking part in a related study at the same institution (confirm with trial site)
