Who might be able to join this trial:

  • You have agreed in writing to take part in the study voluntarily
  • You are between 18 and 75 years old (any sex)
  • Your doctors have confirmed through a tissue sample (biopsy) that you have rectal cancer of a specific type called adenocarcinoma
  • Your cancer has been assessed by MRI scan as being at a middle or advanced local stage (Stage II or III)

  • Your cancer can be removed by surgery
  • You are able to swallow tablets normally
  • You are generally well enough to carry out light daily activities (confirm with trial site)
  • You have not received any previous treatment for your rectal cancer (no radiotherapy, chemotherapy, or surgery)
  • You are planned to have surgery after completing the pre-surgery treatment used in this trial
  • There is no medical reason preventing you from having surgery
  • Your blood tests show that your major organs (such as liver, kidneys, and blood cells) are working normally

Who may not be able to join:

  • You have had an allergic reaction to monoclonal antibody medicines, to a drug called tislelizumab, or to capecitabine (a chemotherapy tablet)
  • You have already received any cancer treatment such as surgery, radiotherapy, chemotherapy, targeted therapy, or immunotherapy
  • You currently have, or have previously had, a disease where your immune system attacks your own body (an autoimmune disease)
  • You have a weakened immune system, including HIV infection, any other immune deficiency condition, or you have had an organ transplant or bone marrow transplant from a donor
  • You have significant heart problems that are not well controlled, such as moderate-to-severe heart failure, unstable chest pain (angina), a heart attack within the past year, or serious irregular heartbeats
  • You have been diagnosed with another cancer in the last 5 years — although certain very low-risk skin cancers or early-stage cervical changes that have been fully treated may still be considered (confirm with trial site)
  • You are currently pregnant or breastfeeding
  • You have other serious health conditions, mental health disorders, substance misuse issues, or personal circumstances that the study doctors feel could affect your safety or ability to take part fully (confirm with trial site)
