Who may be able to join

  • Adults aged 18 or older at the time of signing consent forms
  • People who are well enough to carry out normal daily activities, or who have some limitations but are up and about for more than half of waking hours (as measured by a standard doctor-rated scale called ECOG, scoring 0 or 2)
  • People who have been newly diagnosed with small cell lung cancer (SCLC) that has spread beyond the chest or returned after earlier treatment, confirmed through a tissue or cell sample
  • People whose cancer can be measured on scans according to standard criteria (called RECIST v1.1)
  • People who have had a brain MRI, including those with cancer that has spread to the brain, provided the brain metastases have been treated at least one week before starting the trial drug, symptoms are under control, and any steroid dose is stable and low — or, for untreated brain metastases with no swelling on scans, after discussion with the trial sponsor
  • People whose blood counts and organ function meet minimum levels set by the trial, including healthy enough levels of white blood cells, platelets, red blood cells, and kidney and liver function (confirm specific values with trial site)
  • People whose heart is pumping at 50% or more efficiency, with no significant fluid around the heart and no concerning heart rhythm findings on tests
  • People with an oxygen level of 90% or higher breathing room air
  • People who have stored tumour tissue available (such as a tissue block or slides from a previous biopsy)
  • People who are willing to provide blood samples at set times during the trial
  • People with existing drainage tubes in place (such as a PleurX catheter) for fluid build-up
  • People with COPD who have been on stable supplemental oxygen for at least 6 months may be considered after discussion with the trial sponsor
  • People who are able to understand and follow the study procedures for the full length of the trial, as assessed by the enrolling doctor

Who may not be able to join

Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.

  • People currently taking part in another clinical trial or who have received an experimental treatment within the last 4 weeks
  • People with another cancer (past or current) whose disease or treatment could interfere with the safety or results of this trial
  • People with brain metastases or cancer spread to the lining of the brain that are currently causing symptoms
  • People with serious heart problems — such as unstable heart rhythm, chest pain, a heart attack, or significant heart failure — within the 3 months before starting the trial drug
  • People with active and uncontrolled neurological conditions such as an ongoing stroke, uncontrolled epilepsy, brain blood vessel inflammation, or degenerative brain disease (people with a past history of stroke with no symptoms or deficits in the last 6 months, or controlled epilepsy with no seizures in 2 years, may be considered)
  • People with signs of a serious active infection within 7 days before starting the trial drug, or those currently receiving antibiotics through a drip or injection (may be reconsidered once infection has cleared)
  • People with active hepatitis or HIV infection that is not well controlled — those with certain past or treated infections, or HIV that is undetectable on treatment for at least 4 weeks, may be considered (testing is required)
  • People with a known allergy or severe reaction to any ingredient in the trial drug (tarlatamab)
  • People who have had prior treatment for small cell lung cancer — those with limited-stage SCLC that progressed more than 6 months after completing chemotherapy and radiation may be considered after discussion with the sponsor
  • People who have had major surgery within the last 30 days, or who have not yet recovered from a recent surgery
  • People with an active autoimmune disease or immune deficiency that required systemic treatment or steroids (other than standard hormone replacement) in the past 2 years, or who currently need immune-suppressing medication
  • People with a history of having received a solid organ transplant (such as a kidney, liver, or heart transplant)
  • People with active lung inflammation (pneumonitis) or interstitial lung disease that is not related to infection (those with scarring from past radiation in the treated area may be considered)
  • People with known psychiatric conditions or substance use problems that would make it difficult to follow the trial requirements
  • People who have received a live vaccine within 28 days before starting the trial drug, or who plan to receive one during the trial; COVID-19 vaccination should be avoided in the 14 days before starting treatment
  • People of childbearing potential who are not willing to use effective contraception during the trial and for 60 days after the last dose of the trial drug
  • People who are pregnant, planning to become pregnant, or currently breastfeeding (or planning to breastfeed during the trial and within 60 days after the last dose)
  • Males who are unwilling to avoid donating sperm during the trial and for 60 days after the last dose
  • People with fluid build-up around the lungs, heart, or abdomen that requires repeated draining — though those with a permanent drainage catheter already in place may be considered
  • People with severe breathing difficulties or risk of breathing failure due to tumour pressure that needs urgent medical attention
  • People with any other significant medical condition, treatment history, or abnormal test result that the treating doctor believes would make participation unsafe or interfere with the trial
