Who may be able to join

- People who have had lung function tests done within 6 weeks before their specialist team review, following national guidelines
- People who meet the trial's definition of a "high-risk surgical patient," which means meeting at least one main risk criterion or at least two secondary risk criteria, as listed below:
- Main risk criteria: lung function measured by FEV-1 at or below 50% of expected, and/or lung gas transfer capacity (DLCO) at or below 50% of expected
- Secondary risk criteria (at least two required if no main criterion is met): aged 80 or older; FEV-1 or DLCO between 51–60% of expected; high blood pressure in the lungs (pulmonary hypertension) with a pressure reading of 40 mmHg or above, diagnosed within the past 6 months; or reduced heart pumping function (LVEF at or below 40%), diagnosed within the past 6 months
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
- People who have been declared terminally ill or whose life expectancy is less than one year
- People whose cancer has spread to more than one area of the lung (multifocal disease)
- People with a general health and activity level rated 3 or higher on the ECOG scale, meaning they have significant limitations in daily activity (confirm with trial site)
- People whose tumour is located in a central part of the lung and is not suitable for the type of targeted radiation therapy (SBRT) used in this trial
- People who have previously had surgery on the same lung being considered for treatment
- People who are pregnant or breastfeeding
- People who are unable to understand spoken or written information about the trial and give informed consent
- People with current intravenous drug use or severe alcohol use (more than 25 units per week)
- People who are not able to undergo surgery under general anaesthetic
- People who have previously had radiation therapy to the chest area in a way that could make further targeted radiation unsafe or less effective due to cumulative dose limits
- People who have been diagnosed with interstitial lung disease (a group of conditions that cause scarring or inflammation of the lungs)
