Who may be able to join
- Adults aged 18 or older.
- People who have been diagnosed with advanced lung adenocarcinoma (a specific type of lung cancer) confirmed through a tissue or cell sample.
- People whose cancer has spread to no more than 3 different organs in the body (with any number of individual spots within those organs).
- People who have not yet received any treatment for their cancer, and whose cancer is classified as stage IV (the most advanced stage) according to current international staging guidelines.
- People who have fluid around the lung (pleural effusion), as shown on a chest CT scan or ultrasound; ideally, cancer cells will have been found in that fluid, but if not, imaging must clearly show cancer deposits on the lining of the lung.
- People who, after 8 to 10 weeks of treatment with a medicine called furmonertinib (with or without procedures to drain the fluid), show that their cancer has responded or remained stable on scans, and the fluid around the lung has been adequately reduced to a small amount or none at all (confirm with trial site for exact measurements); a small amount of fluid around the heart may also be acceptable.
- People who have not had any previous radiotherapy to the chest area.
- People whose cancer has a specific genetic change (mutation) in the EGFR gene — either an exon 19 deletion or an exon 21 L858R mutation.
- People who have not previously received any systemic cancer treatment (such as chemotherapy or targeted therapy).
- People who are reasonably well and able to carry out daily activities, with a life expectancy of at least 12 weeks (as assessed using a standard medical scale called ECOG, scoring 0 to 2).
- People who have at least one area of cancer that can be measured on scans according to standard criteria called RECIST 1.1.
- People whose blood counts are at adequate levels to safely receive cancer treatment, including white blood cells, haemoglobin, platelets, and neutrophils meeting minimum thresholds (confirm with trial site for exact values).
- People whose kidney and liver function blood test results are within acceptable ranges, including creatinine, liver enzymes (ALT and AST), bilirubin, albumin, and prealbumin (confirm with trial site for exact values).
- People who have brain metastases (cancer that has spread to the brain) that are not causing any symptoms.
- People who have provided written consent to take part in the trial.
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
- People who have an existing lung condition called interstitial lung disease (ILD), or who had an infection with fever before starting treatment.
- People whose cancer continued to grow (progressed) during 8 to 10 weeks of TKI therapy, or who developed a moderate to severe form of interstitial lung disease (grade 2 or higher) during that time.
- People who have an autoimmune disease that requires long-term use of steroid tablets taken by mouth.
- People with severe anaemia (very low red blood cell levels).
- People who are known to have had an allergic reaction to furmonertinib.
- People experiencing significant breathing-related symptoms — such as chest tightness or persistent cough — that would make it difficult to tolerate radiotherapy.
- People with active hepatitis B or C infection alongside significant liver damage (grade 2 or higher); however, people whose liver function improves to a mild level (grade 1) after appropriate treatment for hepatitis may be considered eligible (confirm with trial site).
- People whose fluid around the lung has not been adequately controlled or has continued to worsen after 8 to 10 weeks of furmonertinib treatment.
- People who have brain metastases that are causing symptoms.
