Who might be able to join this trial:

• People who have been diagnosed with major depressive disorder (a formal diagnosis of serious, ongoing depression) according to standard medical guidelines known as DSM-5.
• People aged 12 to 22 years old who have at least one parent or guardian available to monitor them throughout the 3-month study period.
• People whose depression is rated as moderately severe or above on a standard medical scoring tool called the HAMD-17 (a score of 18 or higher).
• People who have harmed themselves without the intention of ending their life on two or more occasions in the 2 weeks before entering the study, and who have done so on more than 5 days in the past year, meeting specific DSM-5 criteria for this behaviour.
• People (and their guardians) who have given their formal written agreement to take part in the trial.

Who may not be able to join:

• People who use substances such as alcohol or mind-altering drugs in a way that is considered abuse.
• People with a serious physical disability that would prevent them from attending follow-up appointments.
• People who also have another major mental health condition diagnosed under DSM-5 guidelines, such as bipolar disorder, schizophrenia, intellectual disability, dementia, severe memory or thinking problems, or attention deficit hyperactivity disorder (ADHD).
• People who have ever had a severe physical illness, a neurological (brain or nervous system) condition, or a traumatic brain injury that has affected how their brain is structured or works.
• People who are unable to read, understand, or complete the study assessments, or who are unable to cooperate with the research team.
• People who have a pacemaker, metal or magnetic objects in their body, or any other condition that makes a specific type of brain stimulation treatment used in this trial (called TIS) unsuitable for them.
• People who have received certain structured talking therapies (such as interpersonal therapy, dynamic therapy, or cognitive behavioural therapy) or a brain stimulation treatment called TMS in the 3 months before the study begins.
• People whose test results or other factors are considered by the research team to make participation unsuitable (confirm with trial site).
