Who may be able to join

  • The person joining the trial (and/or their legal representative) must have given documented informed consent, and agreement from the participant themselves will also be sought where appropriate.
  • Adults aged 18 years or older.

  • People who have cancer that has spread to the brain are able to participate, but at least one area outside the brain must also be planned to receive SBRT.
  • People whose blood test results show an adequate level of a type of white blood cell called neutrophils (at least 1,000 per mm³).
  • People whose platelet count (cells that help blood clot) is at least 50,000 per mm³, without having received a platelet transfusion in the 14 days before that measurement.
  • People whose liver function blood tests (bilirubin, AST, and ALT) are within acceptable ranges as defined by the trial.
  • People whose kidneys are filtering at an adequate rate (creatinine clearance of at least 30 mL/min, measured by a urine or formula-based test).
  • Women who could become pregnant must have a negative pregnancy test (urine or blood) before joining.
  • People who are capable of having children (men and women who have not been surgically sterilised, or women who have not gone without a menstrual period for more than one year) must agree to use effective contraception or avoid heterosexual activity during the study and for at least 6 months after the last treatment.

Who may not be able to join

Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.

  • People whose doctor plans to stop pembrolizumab at the time they are being referred for SBRT.
  • People who have received a live attenuated vaccine (a type of vaccine made with a weakened live virus) within 4 weeks before starting the study treatment, except for upcoming COVID-19 or flu vaccines.
  • People who are currently using, or have used within the past 14 days, medications that suppress the immune system — with certain exceptions, including: nasal sprays, creams, or inhaled steroids; local steroid injections (such as into a joint); low-dose daily steroids (up to 5 mg of prednisone or equivalent); or steroids used to prevent allergic reactions to treatments or scans.
  • People who have had allergic reactions to medications that are chemically or biologically similar to the study drug.
  • People with a significant uncontrolled medical illness.
  • People who have had an infection requiring antibiotic treatment taken by mouth or by injection within 14 days before starting the study.
  • People who are unable to tolerate a PET scan (a type of imaging scan) even with the help of calming medications.
  • People who have another active cancer that could affect the safety or results of this trial (confirm with trial site).
  • Women who are pregnant or breastfeeding.
  • People with any other condition that, in the treating doctor's judgement, would make participation unsafe.
  • People who, in the treating doctor's opinion, may not be able to follow all of the study requirements for practical or logistical reasons.
