Who might be able to join this trial:

• People who have at least one liver tumour that has been confirmed by laboratory tissue testing to be a type of cancer called adenocarcinoma that has spread (metastasised) to the liver.
• People who have been confirmed by laboratory tissue testing to have adenocarcinoma of the colon and/or rectum.
• People who do not have serious local complications at the time of surgery, such as bowel blockage, severe bleeding, abscesses, or cancer growing into nearby structures.
• People who do not have cancer that has spread to areas outside the liver.
• People whose cancer outside the liver (if present) can be fully treated at the same time as liver surgery or ablation (confirm with trial site).
• People for whom complete surgical removal (with clear margins) of both the bowel tumour and liver tumours is technically possible.
• People who would have enough healthy liver tissue remaining after surgery — generally more than 30–40% of liver volume.
• People who would have at least 2–3 liver sections remaining after surgery, with healthy blood vessels and bile ducts still functioning, and at least one major liver vein unaffected by the cancer.
• People whose liver tumours are no larger than 3 cm in size.
• People who have no more than 3 liver tumours.
• People whose tumours are located at least 1.0 cm away from important nearby structures such as major blood vessels and bile ducts.
• People whose case has been reviewed by a multidisciplinary medical team and found suitable for both surgery and local ablation treatment.
• People who have provided written informed consent to participate.

Who may not be able to join:

• People who have a history of other types of cancer.
• People who need emergency surgery due to complications such as bowel blockage, severe bleeding, or abscesses.
• People whose bowel or liver cancer has grown into the abdominal wall or surrounding organs in a way that makes complete surgical removal impossible.
• People whose liver lesions cannot be completely treated with ablation based on ultrasound and MRI findings.
• People who would have fewer than 2 liver sections or less than 30–40% of liver volume remaining after surgery.
• People who have lymph node involvement that cannot be surgically removed.
• People who have been assessed as having a high anaesthetic risk (ASA grade IV or above) or who have significant limitations in daily functioning (ECOG score of 2 or above) (confirm with trial site).
• People whose cancer outside the liver is not considered suitable for treatment.
• People who have a physical or psychological dependency condition.
• People who are pregnant or breastfeeding.
• People who have an infection that is not under control before surgery.
• People who have participated in another clinical trial within the past 4 weeks, or who have other clinical or laboratory findings that the investigators consider unsuitable (confirm with trial site).
