  • People aged 18 to 70 years old who have been diagnosed with hypertrophic cardiomyopathy (HCM), thoracic aortic disease (TAD), and/or radiation-induced heart disease (RIHD), and who are attending the Cleveland Clinic Main Campus for a full cardiac assessment to plan their care (confirm with trial site for exact diagnosis definitions).
  • The person joining, or someone in their personal support network, has access to a phone, computer, or tablet with an internet connection to complete follow-up questionnaires sent by email.

Who may not be able to join:

  • People who have already had heart surgery related to HCM or TAD, or who are planning to have such surgery (including open chest surgery or a procedure done through the blood vessels) within the next nine months.
  • People who, in the opinion of the research team, are unable or unwilling to follow the requirements of the study.
  • People who are not able to give their own informed consent to participate.
