Who might be able to join this trial:

• People who are over 18 years old
• People who have tested positive for HIV-1 through a specific blood test called a 4th generation Ag/Ab ELISA assay
• People who have been on HIV treatment (antiretroviral therapy) that has kept their virus levels below 40 copies/mL for at least 2 years
• People whose CD4 count (an immune system marker) has been at or above 200 cells/mm³ for at least 1 year
• People with a BMI of 30 or higher, or a BMI of 27 or higher combined with high blood pressure, abnormal cholesterol levels, or type 2 diabetes
• People who understand the study procedures, are able to follow them, and are willing to give written consent to take part

Who may not be able to join:

• People who are unable to follow the study procedures or unable to give themselves injections under the skin
• People whose weight problems are caused by certain hormonal or glandular conditions, such as an underactive thyroid, Cushing's syndrome, problems with sex hormones, certain brain disorders, polycystic ovary syndrome, or a rare insulin-producing tumour
• People whose weight problems are linked to taking certain antipsychotic medications known to cause weight gain, such as olanzapine or clozapine
• People who have used certain diabetes or blood sugar medications — including GLP-1 receptor agonists (such as liraglutide, semaglutide, or exenatide), DPP-4 inhibitors (such as saxagliptin, linagliptin, or sitagliptin), or insulin — in the past 3 months
• People with seriously reduced kidney function, defined by a specific kidney measurement (creatinine clearance) below 30 mL/min
• People who have been diagnosed by their doctor with HIV-related changes in body fat distribution (lipoatrophy or lipodystrophy)
• People with severely reduced liver function, based on a standard liver scoring system (Child Pugh score above 9)
• People with active hepatitis B or active hepatitis C infection, confirmed by specific blood tests
• People with any serious active illness — including any AIDS-defining illness — that the study doctor believes would make participation unsafe
• People with a history of cancer, except those who have been treated for Kaposi's Sarcoma, or people currently receiving chemotherapy or radiotherapy
• People who are currently using illegal intravenous drugs
• People who are already taking part in another clinical trial of an experimental medicine
• People whose screening test results show other abnormalities that the study doctor considers relevant (confirm with trial site)
• People with a known or suspected allergy to semaglutide or any of its ingredients
• People who are currently taking another prescription medication primarily intended for weight loss, such as orlistat
• People who are pregnant or breastfeeding at the time of screening, planning to become pregnant, or unwilling to use contraception for the duration of the study
