Who might be able to join this trial:

• People who have been diagnosed with hepatocellular carcinoma (a type of liver cancer) that has come back, confirmed by a tissue or cell test.
• People who previously had a liver transplant as a curative treatment for their liver cancer.
• People whose returned cancer cannot be treated with surgery or local treatments aimed at a cure.

• People with a general health and activity level rated at 0 or 1 on a standard medical scale (meaning fully active or restricted in strenuous activity but able to carry out light work) (confirm with trial site).
• People with a life expectancy of more than 3 months.
• People whose organs (such as kidneys, liver, and bone marrow) are functioning within normal ranges.
• People with adequate heart function.
• People with no signs of an active, uncontrolled infection.
• People who are willing and able to sign a written consent form.
• People who have recovered from side effects of any previous treatment to a mild or resolved level.
• People who are able to swallow and take oral (by mouth) medications.
• People who agree to use effective contraception for at least 30 days after the last dose of study medication (for women) or 3 months (for men), and women must have a negative pregnancy blood test.

Who may not be able to join:

• People who have had systemic drug therapy or radiotherapy within the last 3 weeks.
• People currently taking part in another clinical trial involving an experimental treatment.
• People with cancer that has spread to the brain, unless it has been stable for more than 3 months.
• People who have had an allergic reaction to cabozantinib in the past.
• People with serious uncontrolled heart or blood vessel problems.
• People with serious stomach or bowel conditions, including those that carry a high risk of perforation (a hole forming in the gut) or abnormal connections forming between organs.
• People who have had major surgery within the 2 months before joining the trial.
• People with certain untreated lung conditions, including air-filled cavities in the lungs or disease blocking the airways.
• People with a tumour growing into a major blood vessel.
• People who have had a significant bleeding episode within the last 3 months.
• People with a serious active infection requiring treatment through a drip, or people known to have HIV or AIDS-related illness.
• People with a serious wound, ulcer, or bone fracture that is not healing.
• People with a condition that prevents proper absorption of food and medications from the gut.
• People with an underactive thyroid that is not well controlled and causing symptoms.
• People who require dialysis (kidney filtering treatment).
• People who are pregnant.
• People with active hepatitis B or C infection in the transplanted liver.
• People found to have cellular rejection of the transplanted liver on a biopsy (a small tissue sample test), based on standard medical criteria (confirm with trial site).
• People whose liver cancer is a specific less common type called fibrolamellar carcinoma or mixed hepatocellular cholangiocarcinoma.
• People with moderate or severe fluid build-up in the abdomen (ascites).
• People currently taking blood-thinning medications at treatment doses.
• People with a history of another cancer, unless they were treated with the intent to cure and have had no signs of that cancer returning for at least 2 years before joining the trial.
• People with any other condition that the trial doctor believes would make participation unsafe or difficult to manage.
