Who might be able to join this trial:

• Imaging results strongly suggest a type of bile duct cancer called perihilar cholangiocarcinoma (pCCA) (confirm with trial site)
• The cancer may involve certain blood vessels near the liver, but must not have blocked the main portal vein outside the liver
• The cancer may involve certain liver arteries in specific locations, as assessed by the medical team
• This must be the first time this cancer has been diagnosed in the person
• The cancer has been assessed by doctors as not suitable for surgical removal, either because of where it is located or because of reduced liver function
• No spread of cancer to areas outside the liver has been found on imaging scans
• A PET-CT scan shows no signs of cancer that has spread outside the liver
• A CT or MRI scan of the chest, abdomen, and pelvis (done within 4 weeks before a specialist meeting at the transplant unit) shows no signs of cancer spread outside the liver
• The person is at least 18 years old
• The person has a good general health and activity level, rated 0 or 1 on a standard medical scale called ECOG
• Blood test results fall within acceptable ranges for red blood cells, immune cells, platelets, liver function, kidney function, protein levels, and a specific antibody called IgG4
• The person has received chemotherapy for at least 6 months, with at least a 10% reduction in the cancer according to standard measurement criteria, and the cancer has not grown or spread at the time of transplant listing
• At least 10 months have passed since the cancer was first diagnosed
• The person must be accepted as a transplant candidate before the cancer shows signs of progression
• At least 12 months must have passed between the date of diagnosis and the date of being formally listed for a liver transplant
• The person has agreed in writing to take part and is expected to cooperate with treatment and follow-up appointments, in line with research and local regulations

Who may not be able to join:

• The cancer involves certain major blood vessels, including the common hepatic artery, the celiac trunk, or the superior mesenteric artery
• The cancer involves the main portal vein
• The cancer involves the large vein that runs behind the liver (inferior vena cava)
• There has been a rupture or break in the lining of the abdominal cavity
• The person has lost more than 15% of their body weight in the last 6 months
• The person has a BMI (body mass index) above 30
• The person has had another cancer, unless it was fully treated more than 5 years ago with no return of the disease
• The person has a known history of HIV infection
• The person has previously had an organ or bone marrow transplant
• The person has substance use issues, or medical, psychological, or social circumstances that may affect their ability to take part in the study
• The person has a known allergy or sensitivity to a medication called rapamycin
• The cancer has previously spread to areas outside the liver
• The person is pregnant or breastfeeding
• The treating doctor has any other reason to consider participation unsuitable
