Who may be able to join

For clinicians:

  • Primary care doctors, nurse practitioners, or physician assistants working at one of the three study locations within the VA health system who are able to complete goals of care conversation notes and orders.
  • Clinicians who have at least 15 eligible patients without goals of care conversation notes at the start of the first stage of the study.
  • Clinicians who have written fewer than 4 goals of care conversation notes in the previous 6 months.
  • Clinicians who regularly attend their Patient Aligned Care Team (PACT) meetings.

For patients:

  • Veterans enrolled in VA healthcare at one of the three study locations who are currently seeing one of the eligible primary care clinicians in the study.
  • People who have been diagnosed with at least one of the following conditions: cancer, heart failure, interstitial lung disease, chronic obstructive pulmonary disease (COPD), end-stage kidney disease, end-stage liver disease, or dementia.
  • People with a Care Assessment Need score of 90 or higher — this is a VA scoring tool that estimates the risk of hospitalisation or death within one year (confirm with trial site).

Who may not be able to join
