Who might be able to join this trial:

• You have been diagnosed by a doctor with chronic rhinosinusitis with nasal polyps (a condition where nasal polyps grow in the sinuses), with or without asthma, and you meet the requirements for using the medication Dupilumab as approved by the FDA.
• You are 18 years of age or older.
• You are willing to give your consent to take part in the study.
• You have insurance that covers the medication Dupilumab, or you are able to get coverage through the "Dupixent MyWay Program" (a patient support program).

Who may not be able to join:

• You are under 18 years old.
• You have or are suspected of having a fungal sinus infection called allergic fungal rhinosinusitis.
• You have or are suspected of having cystic fibrosis.
• Your insurance has denied coverage for Dupilumab and you were also unable to get coverage through the "Dupixent MyWay Program."
• You have taken a short course of steroid medication (such as prednisone) in the last 30 days — however, if you take a low regular daily dose of prednisone (20 mg or less per day), you may still be eligible (confirm with trial site).
• You have taken a different biologic medication (a type of specialist drug) in the last 3 months.
• You have been diagnosed with a condition called EGPA, also known as Churg-Strauss Syndrome (a rare inflammatory condition).
• You are currently pregnant.
• You have a type of non-cancerous growth in the nose called an inverted papilloma.
