Who might be able to join this trial:
- You are 18 years of age or older
- You have been experiencing heart failure symptoms (such as shortness of breath or fatigue) rated as moderate to severe within the last month
- Tests such as an ultrasound, MRI, or nuclear scan have shown that your heart's pumping strength is 40% or less
- A recent heart artery scan (done more than 6 months ago) has shown that a significant portion of your heart muscle is at risk due to blocked or narrowed arteries
- Your medical team believes that a procedure to restore blood flow — either through a stent procedure or bypass surgery — can meaningfully improve blood supply to your heart
- You have been on recommended heart failure medications for at least 1 month (or, if newly diagnosed in hospital, these medications have been started before joining the trial)
- You are willing and able to provide written consent to participate
Who may not be able to join:
- You have already taken part in this study before
- You needed emergency heart support (such as a breathing machine, heart pump, or strong heart medications given through a drip) in the 48 hours before joining
- You had a heart attack within the last month
- You had a stent procedure (PCI) within the last 3 months
- You have a heart valve problem or another heart condition that requires surgery
- You have a high risk of serious bleeding, or your doctors feel you cannot safely take two blood-thinning medications together long-term
- Your doctors expect you to live less than 3 years due to a non-heart-related illness
- There are concerns that you may have difficulty following the treatment plan
- Your health records are not accessible through public health databases (confirm with trial site)
- You are pregnant, or you are a woman who could become pregnant and are not using a reliable form of contraception or have not been sterilised
