Who might be able to join this trial:
- People who live in the Detroit area (defined as those who attended a Detroit-area community event)
- People who are 18 years of age or older
- People whose blood pressure at screening was measured at 140/90 mmHg or higher (without blood pressure medication)
- People whose blood pressure at screening was measured between 130–139 mmHg (the top number), who also have at least one additional heart failure risk factor, such as diabetes (with a specific blood sugar marker of HbA1c ≥ 6.5%) or moderate kidney disease (confirm with trial site for exact kidney function levels)

Who may not be able to join:
- People who have already been diagnosed with heart failure
- People with poorly controlled diabetes, based on a blood sugar marker (HbA1c) of 8.0% or higher from recent lab tests
- People who regularly use insulin or who take more than one diabetes medication
- People who are pregnant or planning to become pregnant within the next year
- People whose blood pressure at screening was severely high — 180 mmHg or higher (top number) and/or 110 mmHg or higher (bottom number)
- People whose heart stress marker (BNP or NTproBNP) measured at screening was above a certain level — the trial site can confirm the specific values that apply
- People whose kidney function level (measured by a test called eGFR) is below 45 ml/min from recent lab tests
