Who might be able to join this trial:
- People over 18 years old who have had a procedure to open blocked heart arteries (called PCI) AND who meet at least one "major" risk factor for serious bleeding, or at least two "minor" risk factors for bleeding, as listed below.
Major bleeding risk factors (one of these is enough):
- Taking blood-thinning tablets (anticoagulants) on a long-term basis
- Having severely reduced kidney function (eGFR below 30 ml/min/1.73m²)
- Having a low red blood cell count (haemoglobin below 11 g/dl), or having had a serious bleeding episode in the past 6 months that required a hospital stay or blood transfusion
- Having a long-term condition that causes ongoing bleeding
- Having serious liver disease (cirrhosis) with high pressure in the liver's blood vessels, along with a serious bleeding episode requiring hospitalisation or transfusion in the past 6 months
- Having a very low platelet count (below 100×10⁹/L), which affects the blood's ability to clot
- Having been diagnosed with an active cancer in the past 12 months (not including common non-dangerous skin cancers)
- Having previously had a brain bleed that happened on its own (at any point in life)
- Having had a brain bleed caused by a head injury in the past 12 months
- Having had a moderate or severe stroke in the past 6 months
- Having an abnormal tangle of blood vessels in the brain (cerebral arteriovenous malformation)
- Having had a major operation or serious physical injury in the 30 days before the heart procedure
- Needing a major operation that cannot be delayed while still on dual blood-thinning medication
Minor bleeding risk factors (two of these are needed):
- Being aged 75 or older
- Having moderately reduced kidney function (eGFR between 30 and 59 ml/min/1.73m²)
- Having a mildly low red blood cell count (haemoglobin between 11–13 g/dl for men, or 11–12 g/dl for women)
- Having had a serious bleeding episode requiring hospitalisation or blood transfusion between 6 and 12 months ago
- Taking anti-inflammatory pain medicines (NSAIDs) or steroid tablets on a long-term basis
- Having had a stroke of any kind not already covered by the major criteria above
Who may not be able to join:
- People who were already experiencing active bleeding at the time they were enrolled in the trial
- People who would not be able to be contacted for follow-up appointments or check-ins (for example, due to changed contact details)
