Who might be able to join this trial:
For patients with heart failure:
- People who have a confirmed diagnosis of heart failure (either the pumping or relaxing type)
- People who have had trouble taking their heart failure medications as prescribed
- People who have a care partner (such as a spouse, child, partner, relative, or friend) who is the main person helping with their heart failure care
- People who are willing to have their care partner involved in helping with their medications
- People who have been on a stable dose of heart failure medications for at least 3 months, following a full heart failure assessment
- People who live in a situation where they are responsible for taking their own medications
- People who are willing to use an electronic pill box called SimpleMed+ to track medication taking
- People who can be reached by phone
For care partners:
- Unpaid family members, friends, partners, or other people who help with the patient's care at least 3 times a week, and who have been chosen by the patient
- People who are willing to take part in the program together with the patient
- People who are 18 years of age or older
Who may not be able to join:
For patients with heart failure:
- People with cognitive impairment that makes it difficult to understand and agree to participate
- People who have been hospitalised in the 3 months before joining the study
- People who have end-stage kidney disease, a terminal illness such as advanced cancer, or any other condition where life expectancy is less than one year
- People with a psychotic illness
- People with current alcohol dependence or other substance misuse
- People who are unable to communicate in English or have another significant communication barrier
- People who are currently taking part in, or have recently taken part in, a similar self-care program within the past year
For care partners:
- People with cognitive impairment that makes it difficult to understand and agree to participate
- People who have a terminal illness
- People who are unable to communicate in English or have another significant communication barrier
