Who might be able to join this trial:

• You are 18 years old or older
• You are seriously ill and admitted to the Medical Intensive Care Unit (MICU) and need a breathing machine, breathing support through a mask or high-flow oxygen through the nose, or medications to support your blood pressure
• You were admitted to the hospital within 24 hours before entering the MICU, and the study would begin on the second or third day of your MICU stay
• You are still on the qualifying breathing or blood pressure support at 9:00 AM on the day you would be enrolled in the study

Who may not be able to join:

• You are not expected to stay in the MICU for at least 48 hours after being enrolled in the study
• You are considered to be at the very end of life or receiving hospice care
• You have a severe long-term brain injury (more than 30 days ago) that prevents you from living independently
• You have any level of recent brain injury that could affect the part of the brain that controls your body clock — for example, a cardiac arrest (confirm with trial site)
• You have a diagnosed sleep/body clock disorder, or you are blind or have a disease of the optic nerve
• You have worked night shifts or rotating shifts in the past year
• You are currently or were recently homeless, in prison, or living in an institution
• You are at higher risk of inhaling food into your lungs due to a stomach or digestive problem, or you were receiving tube feeding before entering the ICU
• You were admitted to the ICU for a serious diabetes-related emergency (diabetic ketoacidosis or hyperosmolar state)
• You have a history of low blood sugar (hypoglycemia) without a documented full recovery
• You have a medical history suggesting an unusually high risk of low blood sugar, such as a known insulin-producing tumor, repeated unexplained episodes of low blood sugar, or severe liver failure
• You were admitted due to a suicide attempt
• You were admitted due to a drug overdose or are going through active alcohol withdrawal
• You have tested positive for SARS-CoV (the virus related to COVID-19)
• For a specific urine test part of the study only: you have or have tested positive for HIV, West Nile virus, Monkeypox, or tuberculosis (TB)
