  • Adults aged 18 or older who are being discharged from hospital after being admitted or seen urgently, where heart failure was either the main reason for the visit or a significant complication that made the stay longer
  • Adults aged 18 or older who have been referred to a cardiology clinic for a first appointment within 1 week of a hospital or urgent care visit related to heart failure
  • People whose heart's pumping strength (measured as ejection fraction) was found to be below 50% in the past 3 months
  • People who had a specific blood test result (NT-proBNP above 900 pg/ml) either during the hospital stay or within 7 days after leaving the emergency department
  • People who have a mailing address — either their own or a caregiver's
  • People who are willing to give verbal consent to participate

Who may not be able to join:

  • People who passed away or left hospital before being medically cleared for discharge
  • People who are unable to assess or describe their own symptoms, such as those with clearly evident dementia
  • People who are unable to use digital health technology or attend follow-up appointments
  • People who have severe heart valve disease
  • People who have already received, or are currently on a waiting list for, a mechanical heart pump (LVAD) or a heart transplant
  • People who have complex heart conditions present from birth, a specific thickening of the heart muscle (hypertrophic obstructive cardiomyopathy), or a protein build-up condition affecting the heart (amyloid cardiomyopathy)
  • People who have severe lung disease with breathlessness during minimal activity, a low lung function score (FEV1 below 1 litre), severely high blood pressure in the lung arteries, or who require oxygen at home
  • People who have severe kidney disease, indicated by a persistently low kidney function score (eGFR below 30 mL/min/1.73m²)
  • People who currently have an active cancer diagnosis
  • People who are receiving palliative care or whose life expectancy is estimated to be less than 6 months
