Who may be able to join
- Adults who have been diagnosed with Alzheimer's disease, related dementias, mild cognitive impairment, Parkinson's disease, or multiple sclerosis, as recorded in their electronic health record.
- Adults who are being diagnosed and treated for one of the conditions listed above and are invited to take part.
- People who are able to sign a written consent form, or who have a legally authorised representative who can sign on their behalf.
- Care partners (such as family members or carers) may be invited to take part in surveys and would provide their own consent to do so.
- Adults who are 60 years of age or older at the time of joining the trial.
Who may not be able to join

- People who have been removed from the trial at the request of the trial sponsor or the trial site.
- People who have had no visits or interactions recorded in the electronic health record system for more than 3 years.
- People in the Engaged Cohort group who have expressed a wish to stop completing the health surveys.
- Care partners in the Engaged Cohort group who have expressed a wish to stop completing the health surveys.
- People in the Engaged Cohort group who did not complete the health surveys within 24 weeks of first being invited.
- People in the Engaged Cohort group who have had a gap of more than 24 months in completing surveys after finishing the initial set.
- People in the Engaged Cohort group who do not meet the disease-related criteria that apply to the main Disease Cohort group (confirm with trial site).
- People listed under the Age Cohort group who have passed away.
- People listed under the Age Cohort group who have been removed at the request of the sponsor or trial site.
- People listed under the Age Cohort group who have had no electronic health record interactions for more than 3 years.

Primary endpoints
To characterize the natural history of disease in patients living with chronic neurological diseases including ADRD, MCI, PD, or MS; To assess safety and effectiveness of disease treatments and treatments for complications of patients receiving various treatments, effectiveness of treatment, and medical events that develop while receiving treatment for ADRD, MCI, PD, or MS.
