Who might be able to join this trial:
- People between 40 and 75 years of age
- People who identify as Black or African American
- People who live in the Cleveland Metro Area
- People who were identified at a University Hospitals (UH) health fair screening as having at least two of the following health risk factors, where at least one of those factors must be related to blood pressure, blood sugar (A1c), or LDL cholesterol levels: a BMI of 30 or higher; a history of smoking; high blood pressure (top number above 140 or bottom number above 80); a blood sugar level (HbA1c) of 5.7% or higher; or an LDL cholesterol level of 130 or higher
- People who are able to complete a heart scan called a coronary artery calcium (CAC) score test
- People who are willing and able to give their consent to participate
- People who are willing to receive care from a UH doctor and within the UH health system
- People who currently have health insurance that covers standard medical procedures
Who may not be able to join:
- People who have already been diagnosed with a heart or blood vessel condition such as coronary artery disease, peripheral artery disease, a heart attack, or a stroke
- People whose blood pressure is very high (top number of 180 or higher, or bottom number of 110 or higher)
- People who have a lung condition that requires them to use supplemental oxygen
- People who are currently receiving treatment for cancer
- People who are pregnant or breastfeeding
