Who might be able to join this trial:

• People who have signed a written consent form agreeing to take part in the study
• People aged 50 or older
• People who have been diagnosed with two or more of the following conditions: type 2 diabetes, chronic obstructive pulmonary disease (a lung condition), ischaemic heart disease (reduced blood flow to the heart), atrial fibrillation (an irregular heartbeat), high blood pressure, chronic kidney disease at a moderate stage (stage 3), or a body mass index (a measure of body weight relative to height) of 30 or above

Who may not be able to join:

• People who have already been diagnosed with one or more of the following: heart failure, disease of the heart muscle (cardiomyopathy), moderate or severe problems with the heart valves, a heart condition present from birth, a previous heart transplant, or certain types of high blood pressure affecting the lungs (idiopathic, heritable, or drug-induced pulmonary arterial hypertension)
• People whose overall health condition, in the treating doctor's judgement, could put them at higher risk during the study or make it unlikely they could complete the study requirements
• People for whom an MRI scan of the heart would not be safe, including those with a pacemaker, a defibrillator, metal fragments in the eye, certain types of surgical clips in the brain, severe fear of enclosed spaces, significantly reduced kidney function (confirm with trial site), a history of a severe allergic reaction to the contrast dye used in MRI scans, or who are pregnant or breastfeeding
