Who might be able to join this trial:

Arm 1:

• Children who have a type of permanent hearing loss (called sensorineural hearing loss) in both ears and who meet the medical standards for receiving cochlear implants, and who are 5 years old or younger when the cochlear implant evaluation starts
• Children whose main language spoken at home is English or Spanish

Arm 2:

• Children who have permanent hearing loss in both ears and who meet the medical standards for receiving cochlear implants
• Children who are 36 months (3 years) old or younger when treatment begins
• Children whose family mainly speaks English at home
• Children who have at least one parent or caregiver willing to take part in the study who understands spoken English
• Children who are exposed to spoken language by at least one parent or caregiver at home (whether through speech alone or a combination of speech and signing)

Who may not be able to join:

Arm 1:

• Children with serious physical movement or thinking/learning difficulties that would make it hard to measure their progress
• Children where only a limited portion of the cochlear implant can be inserted, in a way likely to significantly affect their hearing development
• Children whose hearing loss was caused by bacterial meningitis
• Children whose main home language is not English or Spanish

Arm 2:

• Children with serious physical movement or thinking/learning difficulties that would make it hard to measure their progress
• Children where only a limited portion of the cochlear implant can be inserted, in a way likely to significantly affect their hearing development
• Children whose hearing loss was caused by bacterial meningitis
• Children whose main home language is not English
• Children with a diagnosis or medical condition (separate from hearing loss) that is expected to affect language development
• Children who have a problem with the hearing nerve in the implanted ear, or a severe abnormality in the structure of the inner ear
• Children who already have some spoken language and score above the 25th percentile on a standard language assessment called the CDI (Words and Sentences section)
• Children who have already had more than two months of experience using a cochlear implant before the study treatment begins
